Why did I get a fax from a pharmacy with a Key on it?

Most PA requests start at the pharmacy. The pharmacist begins the PA (with nearly all patient and prescriber information pre-populated), but leans on the provider to complete it and send to the plan for a determination.

To complete one of these pharmacy-initiated requests:

  1. Log into or create your account.
  2. Click “Enter Key” on the left side of your Dashboard.
  3. Enter the key, patient last name and DOB indicated on the fax.
  4. Complete any remaining information and click “Send to Plan” in the upper left corner of the page.

Note: The BIN, PCN and RxGroup numbers will yield the most accurate results. If more than one form populates, choose the one that best fits the patient’s circumstance.
